Lifeline Community Care

Support that helps people live the life they choose.

Care and support in the community for adults and young adults across Jersey, shaped around each person.

Support built around the person

We start by getting to know the person and the people around them: what matters to them, what they'd like help with, and what they'd rather do themselves.

Together we agree what good support looks like and write a personal care plan. We review it regularly, so it keeps up as life changes.

Looking for work in care? We're always keen to hear from kind, reliable people. We'll train you in the rest. Work with us.
